We advocate that each client complete and participate in as much of their personal care so they remain as independent as possible, but when sensitive and potentially hazardous tasks become challenging, often times a little help from a personal care assistant can prevent injury. As a non-medical alternative to in home health care agencies, Sincere Home Care offers home care services to assist older adults with personal care.
Personal Care: A Confident Start to the Day
When you or a loved one wake, often the first task at hand is using the bathroom. A referred personal care assistant can assist with using the restroom, showering, brushing the teeth, and then getting dressed for the day.
Personal Care, for Less Mobile or Bedbound Adults:
A referred personal care caregiver can assist with re-positioning in bed to avoid bed sores, bed pans, adult briefs, a sponge bath, and even range of motion activities.
The personal care program is designed to help frail, health impaired, older persons locate and select the services that can help them continue to live in their homes and stay in the community. This program is offered by the Department of Social Services.
An evaluation of needs is done in the home by a qualified staff person or team. After the needed services are identified, a case manager will help locate and arrange for the services and ensure the individual receives the needed services at the right time. Such persons may be eligible for gap filling service dollars if they meet established financial standards and other eligibility criteria. There is no cost for the individual.

- Be medically at risk of institutionalization
- Be moderately or severely disabled
- Live in Frederick County
- Need assistance with several daily activities such as bathing or meal preparation
Personal Care Program
The Personal Care Program provides in-home aide services to individuals 60 years or older who require assistance with activities of daily living such as bathing, dressing, and meal preparation. In-home aides may also provide chore or homemaker services, as well.

Personal Care Supervision:
All nursing personnel work in accordance with a written assignment prepared by a licensed registered nurse. The nurse makes an initial on-site visit when care is started.
From the moment you contact us, we begin to consider your specific needs. Based on your doctor’s order and on information obtained by our registered nurse at your initial consultation, we develop a Custom Care Plan tailored to your specific needs. After reviewing the plan with you and modifying it as you see fit, we provide the most appropriate caregiver possible. We do more than provide a physical presence in your home, we try to match personalities. If you have concerns about a caregiver, we can promptly arrange a replacement.
Caregivers keep extensive notes about the care they provide. The notes are kept at your homes and are available any time. The Nursing supervisor and/or other scheduled registered nurses make regular unannounced visits to client’s homes to ensure that the caregivers are following the specifics of the Custom Care Plan.
